 Complete the ConXus Steps Action Plans to Receive Incentives!

ConXus Steps - Paramount Members

1. Under New User Registration, click on Register Here (EVEN IF YOU REGISTERED IN THE FALL, YOU WILL NEED TO REGISTER AGAIN).  Read the Terms of Use and click on I Agree. Next, enter the requested New Member Registration information required. Have your Paramount member ID number ready.

2. After you are registered click on the MyPlace Health Risk Assessment link.

3. Click on My Wellness Center in the box at the left.

4. Here, click on Health Risk Assessment in the box.  The assessment takes approximately 20 minutes to complete.  Or, click on Action Plans, choose one and begin.

5. Click here to begin.

ConXus Steps - non-Paramount Members 

1. FOR NEW USERS, click on Register Now.  Read the Terms of Use and click on I Agree. Next, enter the requested New Member Registration information required. Spring is your Company ID, User ID Number (Call Cathy Semonin @ 419-867-5603 to receive your User ID..

2. Click on My Wellness Center in the box at the left.
